I have a Frenum piercing, didn't hurt much on insertion, similar to thickness of nipple piercing, depends on your pain tolerance. If you're afraid of needles or have pain with blood draws that would be an indicator. Frenum is on the underside of the penis and is hardly noticeable when flacid, you can put more than one in and a few are referred to as a ladder. I have never had a complaint nor incident with it and you can easily slip a condom over it. No problems with hygiene never had an infection of any kind. It does build up a little goo on either side of the bar (lube/semen/etc). I clean it with an alcohol prep pad. One drawback is the recommendation of no sex for six weeks as it heals after initial piercing. I used a condom after a few weeks and it kept it in place and prevented any contamination. When completed healed you can twist/pull without any pain. If you take it out for any reason don't leave it out for too long, a few hours at most, as it closes up pretty quickly. Make sure you get a tapered pushrod to reinstall it, makes it much easier. You will trigger an airport scanner with it and require a pat down.
